Upon selecting your membership package and completing the registration process, our team will promptly send you the intake paperwork. You will be seen within 1-2 weeks for a one-hour intake to delve into your goals, needs, and medical history. This conversation forms the foundation for crafting a personalized health plan that aligns perfectly with your unique requirements. At Enlightened Family Medicine, there are no hidden costs for visits, telehealth services, or access to your dedicated physician. Moreover, upon completing a one-year commitment, we offer a refund of the registration fee.
